The Opportunity

Are you passionate about making a difference in the lives of older adults and people with complex needs? We are looking for a dedicated Assessment Officer to assist clients and their carers through the Commonwealth Home Support Program and the Home and Community Care Program for Younger People.

In this impactful role, you’ll support clients in staying engaged with their communities and living independently in their homes longer. Your expertise will help ensure they receive the necessary support and services tailored to their individual needs.

Key Responsibilities:

  • Conduct comprehensive Home Support Assessments and develop Support Plans using a wellness and reablement approach.
  • Maintain a deep understanding of the My Aged Care referral portal and various service options available to clients.
  • Collaborate with stakeholders, participate in networks, and attend relevant meetings to enhance service delivery.
  • Ensure accurate data capture and compliance with Council and funding body reporting requirements.
  • Identify opportunities for continuous quality improvement and actively contribute to program reviews.
