About the Job

There is an exciting opportunity with Esperance Home Care for a Care Partner - Trainee. This position is to gain experience working alongside the Care Partners to coordinate the delivery of services to older people and younger people with a disability, living within the Shire of Esperance.

Responsibilities include, but are not limited to:

• Monitor service referrals and respond within the designated time requirements.
• Check for referrals, meet with and complete the required paperwork for clients commencing with services.
• Work with clients to plan and implement a program of care which is within the guidelines of the relevant funding and budget available.
• Undertake training and relief as required, within the Scheduling role

This is a fixed term, part time position working a minimum of 30 hours per week.  

About you 

In order to succeed and thrive in this position, you must have:

• Knowledge and/or experience within the aged care or disability services industry and a strong commitment to working with older people and younger people with a disability (would consider other related disciplines and a willingness to learn).
• Post secondary qualifications
• Willing to enrol and ability to complete the Certificate IV in Ageing Support or equivalent qualification.

Essential Requirements

• You must be an Australian citizen or permanent resident (sorry, no working holiday or temporary visas).
• Unrestricted Drivers Licence
• Ability to pass a pre-employment medical and drug test and able to obtain a Police Clearance and NDIS Check
